This seems hypocritical, given that most nursing homes and other care facilities often state that their mission statement is to provide comfort during this difficult period of their lives. The fact that about 20 percent of all patients with advanced dementia are put in this situation is extremely troubling. Oftentimes, the move or transition is completely bureaucratic in nature. One researcher concluded that this marks one of the greatest flaws and inefficiencies within our healthcare system. The same outcome could be achieved by keeping the patient within their original nursing home or advanced care facility. At this late stage in dementia treatment, there is very little that can be accomplished from a medical standpoint and comfort and familiarity should be highlighted, rather than sending these patients in for a treatment that in the vast majority of cases will do very little good.

Treatment at this stage is usually unnecessary and it raises the risk of medical error, and thus does more harm than good to many patients. This also poses an interruption of the routine that has already been established for these dementia patients. Routine is especially important for peace of mind, and this practice has been proven to be counterproductive. What little familiarity has been gained in their final days is thrown away and the patient suffers because of this.

The study went on to say that Medicare’s payment system might be one of the big causes of these burdens. In some instances, a nursing home is eligible for as higher payment when patients have been placed within a hospital for three days. The nursing home patient will then qualify for skilled nursing services made payable by Medicare rather than Medicaid when they return to the nursing home. Medicare pays more than Medicaid does, thus giving the nursing home a bonus of sorts. These incentives are built into these government sponsored health plans as guidelines for determining just how a patient is cared for and how that care is paid for, but they are being exploited in some cases by nursing homes.

There is no easy solution to this because utilizing these loopholes, although greedy in nature, is completely legal, albeit not necessarily ethical. The patient’s best interests and comfort should be the nursing home’s primary interest, not their bottom line.
